NOTICE TO CONSUMERS — TEXAS MENTAL HEALTH SERVICES

In accordance with Texas House Bill 4224 (89th Regular Session, effective September 1, 2025) and Section 181.105 of the Texas Health and Safety Code (Required Information Regarding Consumer Access to Health Records and Complaints), the following information is provided for consumers of mental health services.

 

Requesting Your Health Care Records

You have the right to request access to and obtain copies of your health care records maintained by this provider.

To request your records: Submit a written request to this practice or your treating clinician (via email, secure client portal, or signed letter).

 

Include the following in your request:

  • Your full legal name, 

  • Your date of birth

  • A clear description of the specific records you are requesting

  • Your contact information
     

Your request will be processed in accordance with applicable Texas law, the Texas Medical Records Privacy Act, and applicable privacy regulations including HIPAA. Reasonable, cost-based fees for copies may apply as permitted by law. If you have questions or need assistance with your request, please contact our office.

 

Texas Behavioral Health Executive Council (BHEC)

This provider is licensed and regulated by the State of Texas. The BHEC regulates licensed mental health professionals including Licensed Professional Counselors (LPCs) in the state of Texas. You may contact BHEC to verify a license, ask questions about professional standards, or obtain information about filing a complaint.

 

Filing a Consumer Complaint

Complaints may address issues such as failure to provide access to records, professional misconduct, billing practices, or other service concerns. Filing a complaint typically requires submission of a written and signed complaint form and may involve authorization to release relevant confidential information for investigative purposes. If you wish to file a complaint regarding your experience with mental health services or believe your rights under Texas law have been violated, you may do so as follows:
